Medical Records Release
If you would like to request that your medical records be sent from Clinic 5C to another healthcare provider, or to authorize another provider to send your records to Clinic 5C, please download and complete the Medical Records Release form.
If you would like to request your medical records be sent to yourself, please complete the Medical Records Release form and indicate yourself as the recipient.
Once completed, you may submit the form to Clinic 5C by fax, by mailing a printed copy to our office, or by sending an encrypted email to medicalrecords@clinic5c.com. Please note that any email correspondence with your records will also be encrypted for data protection purposes.
